Medication reviews and chronic disease support
A medication review is a one-on-one conversation with your pharmacist about everything you take and how it is working for you.
What we look at
- What each medication is for and how to take it
- Side effects, interactions and possible duplicates
- Timing, missed doses and anything that is hard to manage
- Over-the-counter products, vitamins and herbal supplements
Support for ongoing conditions
If you live with a long-term condition such as diabetes, high blood pressure, high cholesterol or asthma, we can help you understand your medications, use devices correctly and prepare questions for your doctor or care team.
What to bring
Bring everything you take, including prescriptions, over-the-counter products, vitamins and supplements, in their original containers if possible.
Good to know
A review does not replace a visit with your doctor. If we find something that needs a change, we will work with your prescriber.
This page is general information and is not medical advice. Please speak with your pharmacist or doctor about your own situation.
